Rdh Chart Notes Template

I have designed this easy to use template for notes for each appointment.  Rdh's can use this for Complete exam, scaling, whitening, etc.  It is very thorough. It helps save time, as it can be uploaded for digital use, as well as printed on sticker backed paper for those still using paper charts.
